Personal Action Toward Health (PATH) for Diabetes

PATH (Personal Action Toward Health) provides the skills and tools needed to manage chronic health conditions. People who participate in PATH workshops are better equipped to face the daily challenges of living with one or more chronic conditions.
PATH is a six-week series facilitated by two trained leaders who have chronic conditions themselves. There is a PATH workshop specifically designed for people with Type 2 diabetes.
Subjects covered include:
- Techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration
- Appropriate exercise for maintaining and improving strength and endurance
- Healthy eating
- Appropriate use of medication
- Working more effectively with health-care providers
Participants will make weekly action plans, share experiences and help each other solve problems they encounter in creating and carrying out their self-management program. There is no charge for this workshop, which is sponsored by Beaumont Hospital Troy, Area Agency on Aging 1B, and Michigan State University Extension.
