The Stanford Chronic Disease Self-Management Program (CDSMP) is a six-week workshop conducted in two and a half hour sessions in community settings.
Who should participate: PATH is designed to benefit adults with chronic or long-term health conditions including arthritis, heart disease, diabetes, emphysema, asthma, bronchitis and depression. Family members, friends, and caregivers are also encouraged to attend the workshop. The workshop sessions go from 1: 30 to 4 p.m. on Feb. 4, Feb. 11, Feb. 18, Feb. 25, March 4 and March 11.
