The Stanford Chronic Disease Self-Management Program (CDSMP) is a six-week workshop conducted in two-and-a-half-hour sessions in community settings.

Who should participate: PATH is designed to benefit adults with chronic or long-term health conditions including arthritis, heart disease, diabetes, emphysema, asthma, bronchitis and depression. Family members, friends, and caregivers are also encouraged to attend the workshop. PATH workshop sessions will meet on Thursdays from 1 to 3:30 p.m. on Jan. 31, Feb. 7, Feb. 14, Feb. 21, Feb. 28, March 7 and March 14.
