National Diabetes Prevention Program Lifestyle Coach Training

The National Diabetes Prevention Program is an evidence-based program through the Centers for Disease and Prevention Control (CDC). Lifestyle coaches will work in small groups with adults that are at high risk or diagnosed with prediabetes to adopt healthy lifestyles that include healthy eating and incorporating physical activity. Coaches will provide support, encouragement, and information to participants and work through challenges that may arise providing an environment that encourages self-management for the participants health and well being. Lifestyle Coach certification for this program is based on:

  • Completion of the full two-day training
  • Commitment to facilitating each program for one full year (12 months)
  • Commitment to follow the curriculum
  • Be enthusiastic and positive
  • Ability to facilitate a small group
  • Ability to communicate with people efficiently

Pre-registration for this training is required. Please complete the Organizational Capacity Assessment form prior to registering. It is strongly encouraged that lifestyle coaches be associated with an organization that is recognized or seeking recognition for the National Diabetes Prevention Program through the Centers for Disease Control and Prevention.
