DIABETES PATH (Personal Action Towards Health); also known as Healthy Living with Diabetes

DIABETES PATH is an evidence-based workshop for people who have diabetes. Based on the Chronic Disease Self-Management Program developed at Stanford University, the Diabetes PATH workshop meets for 2.5 hours a week for six weeks. Classes are highly participative, and mutual support and success build participants’ confidence in their ability to manage their health condition and maintain active and fulfilling lives. The series is facilitated by two trained leaders in a classroom style, but most of the learning comes from sharing and helping others with similar challenges. Topics covered in this six-week series include:

  • Dealing with the symptoms of diabetes
  • Healthy eating
  • Physical activity and moving more
  • Emotional ups and downs
  • Medication usage
  • Communicating with healthcare providers, family members, and friends
  • Problem solving

This series will help you learn ways to take care of your health through setting goals that are achievable, finding support and help for problems, and managing stress. Diabetes PATH (Personal Action Towards Health) provides the skills and tools needed to manage Type 2 diabetes. People who participate in PATH workshops are better equipped to face the daily challenges of having diabetes. Participants make weekly action plans, share experiences, and help each other solve problems encountered in creating and carrying out their self-management program. There is a $10 charge for this workshop, which includes the book Living a Healthy life with a Chronic Condition, as well as light snacks at each session. (Scholarships are available for those who could otherwise not attend the class.) The series is sponsored by UPCAP and Michigan State University Extension.
